Double-side sheets pre-printed sheets calibrated with OHI Cards. They contain a detailed summary of a patients hygiene visit.  The OHI Sheets contain information regarging a patient's arrival time, new or recall, name, last cleaning date, medical history update, oral hygiene status, periodontal status, oral pathology, assessments, treatment rendered with all national ADA codes, carbon copy of the OHI Card, Misc section and an other section.  This information is concise and clearly  layed out for treatment notes to be wrote up perfectly every time in less than a minute regardless if they are a temp or new hire.  All information is input with minimal text or boxes to check.
